Stouffville company fined $150K for workplace fatality
A Stouffville-based glass company has been fined $150,000 as a result of its role in an incident that led to a worker’s death.
Stouffville Glass Inc pled guilty to a violation of the Occupational Health and Safety Act in a Toronto courtroom. The company will also pay a 25% victim-fine surcharge.
The incident occurred on June 28, 2021 at the site of construction of York University’s Continuing Studies Building on Keele Street in North York.
Stouffville Glass Inc. was subcontracted by the constructor to complete the exterior and interior glass installation, as well as the installation of exterior aluminum panels.
On the day of the incident, a supervisor employed by the company was fatally injured after an exterior building panel fell while being hoisted into place at the sixth floor of the project.
An investigation by the Ministry of Labour, Immigration, Training and Skills Development determined that taglines used to control the motion of the panel on the crane were not attached to the load, but instead were attached to a power grip used to hoist the load.
