Formwork company fined $140k for 2021 incident
A concrete forming company based out of Cookstown has been fined $140,000 for its role in an incident that led to a worker’s injuries in 2021.
Resform Construction Ltd., which performs concrete forming, masonry and architectural precast concrete services in the high-rise residential sector, pled guilty in a Toronto courtroom for its role in the incident. In addition to the fine, it will pay a 25-percent victim fine surcharge.
The incident occurred on a high-rise project at 1215 York Mills Road in Toronto on November 11, 2021.
Resform Construction Ltd. has been contracted to perform concrete formwork in the construction of two residential towers, one 24 storeys and the other 12 storeys high.
On the day of the incident, two of Resform’s workers were preparing the ground floor air shaft between two buildings for the installation of forms around a steel rebar cage.
While the workers were taking measurements to determine the required materials for installing the forms around the rebar cage, the rebar cage fell on one of the workers, resulting in critical injuries.
An investigation by the Ministry of Labour, Immigration, Training and Skills Development determined that the rebar cage collapsed and fell onto the worker because it was not secured in place. The rebar cage was free standing in place and the two tie wire connections to the rebar cage were not secured to the existing structure.
